According to the Milaca School Website:

Physicals “Physicals must be on file in the Community Education/Activities Office for all students participating in an activity. Physicals are valid for three years, but individuals may choose to have an exam every year or every two years. The cycle for physicals is 7th grade and 10th grade.”
