New Patient Appointments:

Dr. Christine is accepting new patients, but has limited time due to the current patient load.  You may have to wait a few weeks to get established in her care. There are typically no same day new patient appointments. 


Steps for New Patients:

1. Contact Dr. Christine for a New Patient Appointment
Options:

a) Call or Text 320-983-2333.  She will return your call or text when she isn’t with patients. 

b) Email: staff@naturalelementshealth.com

2. You will be sent Intake Paperwork to your Email to be completed.

a) Receive paperwork via your email.  Complete and Submit before your appointment electronically and HIPAA compliant.  All information remains confidential.

b) Can’t complete paperwork online?  We have paper copies we could email or mail. Return to us via email staff@naturalelementshealth.com, mail to Natural Elements Health 14094 9th Ave S E,  Milaca, MN 56353 or drop by the clinic.

3. We will see you at your New Patient Appointment!

Non-parent or guardian –
Bringing a minor child requires a signed authorization note from parent or guardian allowing you to bring the child to our clinic.


Forms of Payment

We do not work with your health insurance companies.

You are more than welcome to request a receipt called a SUPERBILL with proper coding to send in on your own to apply charges for our services toward any possible deductibles you may have in your health care plan.

We accept cash, checks, HSA/FSA, as well as most debit and credit cards.

Cancellation Policy:
You must give a 24 hour notice to cancel your new patient appointment.
If a 24 hour notice is not given you will be charged the rate of service.
